Neurology Care Navigation & Family Support

Neurological illness affects the whole family. We provide independent neurological education and care-navigation support to help families better understand complex neurological conditions and prepare for conversations with their treating medical team.

How we help

Understanding is the first form of care.

When someone you love has a stroke, a dementia diagnosis, a seizure disorder or an unexplained neurological illness, the hardest part is often not the treatment. It is understanding what is happening, what the options are, and what to ask. A dedicated session with a neurologist, focused on you as the family, can change that.

  • Plain-language explanation of a diagnosis, test results or imaging
  • Help understanding the treatment options that have been offered
  • Preparation for hospital, rehabilitation or clinic conversations, including a written list of questions
  • Guidance on what to watch for and when to seek urgent care
  • Orientation to the kinds of support, services and specialists that may be relevant
  • A second opinion for the patient, when the patient is able to participate and is located in a state where Dr. Al-Habib is licensed
Common situations
After a strokeUnderstanding the type of stroke, recovery expectations and prevention
A new dementia diagnosisWhat the diagnosis means, what to expect, and how to plan
Seizures and epilepsySafety, medications and what an event means
A complex hospital courseMaking sense of many specialists and many opinions
Unexplained symptomsOrganising the story so that the next visit is productive
Scope of this service. Education and care-navigation sessions are consultative and educational. They do not establish a physician–patient relationship with your family member, do not replace the care of the treating team, and do not include diagnosis, treatment orders or prescriptions for a person who is not our patient. Where the patient wishes to become a patient of the practice, a separate consultation is arranged. Final scope language is subject to review by the practice’s healthcare attorney.
